Connections Service

Connections is an innovative and creative service which assists individuals to fulfil their dreams and aspirations through person centred support. The service has evolved to support individuals whose needs can not be met in traditional services. It responds to people who have complex health needs, requiring constant specialist intervention to enable the individual to take a full part in their chosen activities. Other individuals may experience communication difficulties, which can lead to difficult and challenging behaviours. Focusing on the individual, the staff team is able to create, support, develop and maintain meaningful and valued roles within the individual’s local community.

Connections participate in a wide range of activities, be they vocational, recreational or educational. Flexibility of support is a key element; some people start their days from home, others travel into a community base. The support is as diverse as the people involved.

Individuals have access to a base, which is used as a secure place providing important support for both the individuals and team members. The base holds specialist care facilities as well as providing a secure and controlled environment allowing individuals to develop skills which can later be transferred into their local community.

There are three Connections Teams each supporting twelve to fifteen individuals in Whittle-Le Woods, South Ribble and Chorley.
